Would you recommend sitting down and reading first aid at any point in prep? If so, when and how (i.e., meticulously understand every fact, skim, etc.).

Yes, I would recommend sitting down and reading through all of First Aid, but not in the way that most people do. I suggest skimming FA three to five days (depending on your reading speed) prior to your test date. This way you can have FA in your "local memory," so to speak. Whatever you just read in FA prior to this, you will retain very little of, but you'll be able to hold on to some of those facts for a few days.
I also suggest cross-referencing FA based on questions you get wrong in UWorld. These are the parts of FA you need to read more carefully (taking notes). For those who choose to make their own Anki cards (which I highly recommend), I suggest including screenshots from FA (key images or tables) into those cards, using a PDF of First Aid.
I also occasionally assign select chapters of FA (to be read carefully, taking notes) based on very weak areas (subjects with percentile scores lower than 40th in UWorld).
